PROJECT OVERVIEW

Depression is the leading cause of disability worldwide and accounts for about 40% of the disability-adjusted life years (DALYs) attributable to mental and substance use disorders. Present estimates suggest that the prevalence of depression in India is 2.7%, and the treatment gap stands at 85%.

 

Implementation of Evidence-Based Facility and Community Interventions to Reduce Treatment Gap for Depression (IMPRESS) aims to reduce this treatment gap by addressing both supply and demand side barriers by combing two strategies previously developed by Sangath. The supply-side will be targeted by scaling up the Healthy Activity Program (HAP), a manualised psychological treatment based on behavioural activation, which was shown to reduce symptom severity and increase remission among participants with depression in Sangath’s PREMIUM study. A part of the PREMIUM study was the VISHRAM program, which was a community-based mental health intervention with a gal to increase mental health demand through the enhancement of mental health literacy. IMPRESS builds on the work done previously in the PREMIUM study and the VISHRAM program.

PROJECT GOAL

Formative phase:

  • To adapt and test the strategies of the HAP delivery at the healthcare facility by the facility staff and in the community by the community stakeholders (panch members, Auxiliary Nurse and Midwives, social work volunteers, etc.)
  • To assess the feasibility and acceptability of these procedures before the main trial.

 

Trial phase:

  • To evaluate the impact of the feasible and acceptable procedures.
  • To enhance the demand for, and improve the outcomes of, the Healthy Activity Program or HAP delivered by non-specialist health workers (known as Counsellors) in primary health care.

KEY IMPACT

  • This program will be the first systematic attempt in LMICs (lower- and middle-income countries) to scale up the psychological treatment in primary care, supported by evidence.
  • The program will also implement evidence-based community intervention to improve quality utilisation of the psychological treatment.
  • IMPRESS also aims to address one of the major unanswered challenges in global mental health: the methods, acceptability, effectiveness, and cost-effectiveness of integrated community and primary healthcare-based approaches to reduce the treatment gap for depression.
